Care Manager

A Care Manager is a dedicated professional who helps individuals and families navigate the eating disorder care system. Rather than trying to piece everything together alone, you have a single point of contact who learns about your situation, explains available options, and offers guidance as you move through assessment, treatment, and recovery.

Transitioning from a higher level of care (HLOC) to home or a lower level of clinical support is a major milestone, but it often brings a mix of relief and anxiety. The ConnectED Caregiving Program is designed to bridge that gap, ensuring families feel equipped, supported, and organized. Over a 90-day period, families are paired with a Family Peer Mentor—someone with lived experience who walks alongside you to help identify needs, navigate obstacles, and celebrate victories.

Join us for an informative virtual workshop every Thursday at noon, where we will provide guidance on understanding and supporting a loved one diagnosed with an eating disorder. Click here to let us know you’re coming.

In collaboration with EquuStrong, we offer equine-assisted therapy blended with evidence-based eating disorder care. The Hooves & Hearts Program strengthens connection, communication, and trust within and between families supporting a loved one in eating disorder recovery.

Food for the Table is a hands-on, community-based experience that helps people rebuild confidence and connection through learned skills, exposure work, and preparing meals together.

Life coaching helps you clarify your life values and identify and achieve your life goals, and is especially helpful during life transitions. We offer group-based and individual life coaching.
